Introduction
The information on this form is collected under the authority of the Employment Standards Act (the “Act”). The information provided will be used to resolve your complaint and to determine if there were any contraventions of the Act. If you are no longer employed, you must file your complaint within six months of when your employment ended. The last year of your employment will be looked at. If you file a complaint against an employer you still work for, the year before the date you filed will be looked at. Your personal information is collected in accordance with the Freedom of Information and Protection of Privacy Act. The information you provide may be disclosed to provincial and federal authorities for the purposes of law enforcement and jurisdiction, and may also be shared with third parties for the purpose of investigating your complaint.
